Where most providers land after the lab
You see the bulge on exam. You know something is prolapsed. But then the clinical questions start. Does this patient need a pessary? Do they need pelvic floor muscle training? Do they need both? And if they already have a pessary, why are they still leaking?
That hesitation is not a sign that you are not a good provider. It is usually a sign that you were never actually trained to manage prolapse with confidence. Most providers receive a quick lecture, a brief overview of assessment, and maybe a few minutes of exposure to pessaries, then are expected to translate that into real-life clinical decision-making. But prolapse care requires more than identifying that "something is down." It requires understanding the full assessment, the contributing factors, the patient's symptoms, the role of pressure management, pelvic floor function, tissue support, pessary selection, and how all of those pieces work together.
Without that training, many providers either refer out for pessary fitting or avoid pessary care altogether. Meanwhile, these patients are not rare. Some degree of pelvic organ prolapse is seen on exam in roughly 41 to 50% of women, even though far fewer report symptoms. Urinary leakage is also incredibly common, with urinary incontinence affecting a large percentage of adult women and stress urinary incontinence being one of the most common types, especially as women age, after childbirth, and through midlife and menopause. These patients are already in your caseload, asking questions you may not feel fully prepared to answer. Questions like, "Why am I still leaking with the pessary?" "Why did my leaking get worse now that my prolapse is better supported?" "Do I still need pelvic floor therapy if I have a pessary?" "Can pelvic floor muscle training help my prolapse?" and "What type of pessary is right for me?"
